I found the following information:
Loren was killed when the Huey helicopter they were in was shot down during an ARA mission near FB Tomahawk by extremely heavy small arms fire during attack on enemy position North of Da Nang.

Those in the helicopter were:

WO1 John W. Brinkmeyer, Pilot
LTC Roger J. Bartholomew, Co-pilot
SP5 Loren D. Martinsen, Crew Chief
SP4 Ernest L. Barber, Gunner
COL Edward B. Vogel, passenger

All were aboard a UH-1C Huey helicopter #66-00611 that was shot down over Thua Thien, South Vietnam. Four crewmen from the 77th Artillery Regiment of the 101st Airborne Division; WO John W Brinkmeyer, LTC Roger J Bartholomew, Spec Loren D Martinsen, and Spec Ernest L Barber. Col. Vogel was a passenger.
