Salt Lake City, UT-Jackie passed away December 5, 2013 surrounded by her daughters. She was born September 18, 1940 in Parowan, Utah.
She is preceded in death by: her parents, John Mortensen & Buryle H. Mortensen; and her partner Jay Mason.
Her surviving loved ones are: sisters, Annette Lagerstrom and Sheila McCloud; brother Frank Adams; daughters, Sydna Hammond, Tori Richins, Heather Nay, Carmen Eccles, Jaime Law; and many loved grandchildren, Brandi, Austin, Parker, Dillon, Kayden, Tieson, Brianna, Eryn, and Jordyn.
Jackie was the Strength and Force that held the family together. Yet even with the trials and hardships that she encountered throughout her life she still managed to smile. Everything made her soul stronger. With every bumpy road and twisted turn she learned how to survive.
She is our HERO
Jackie's daughters and family want to give Millcreek Hospice our gratitude and warmest thank you.
Veronica, Ximena, Debbie and Heather as well as the rest of the staff, your guidance, support and amazing care of mom will never be forgotten.
