James Puka, 16, whose home was in Ashland, Ohio, died at 1 p.m.yesterday in Samaritan Hospital, Ashland, Ohio. Leukemia was reported to be the cause of death.
He is survived by his parents, Mr. and Mrs. Anthony Puka, a sister, Donna Jean and a brother, Thomas, both at home.
Funeral services will be Friday at 9:30 a.m. in St. Edwards Church, Ashland, Ohio. Friends may call at the Heil Robbins Funeral Home today and tomorrow from 7-9 p.m. Burial will be in the Ashland Cemetery.
